We are currently looking for a Case Manager for our Residential Program. The Case Manager directs patients through substance abuse treatment through interactions with both CHC Addiction Services and in the community. Duties include: Completing appropriate case recording requirements; Communicate with patient treatment providers; Provide patient access to individualized treatment; Promote patient participation in outreach activities; Present patient cases weekly in clinical supervision meetings and seek additional professional consultation as needed.
Minimum Requirements: Bachelor’s Degree (Relevant professional experience may be considered in lieu of a degree), CDCA or other license or credential that conforms to OhioMHAS regulations pertaining to counseling services, and clean driving record.
